Study Abroad Health & Safety Policies
Required Travel Forms
All students participating in international experiences are required to submit an Application to the Office of Study Abroad, as well as appropriate travel forms.
Required travel forms for Faculty-Led Program Participants

- Release Form - This form gives a student's informed consent of studying abroad
- Health Information Form - this form provides important information about a student's health
- Emergency Medical Treatment Authorization Form - this form allows emergency medical treatment in case a student cannot give consent or in case a parent, guardian, or spouse cannot be located to give consent for emergency treatment.

Pre-Departure Orientation
All students participating on faculty-led program are required to participate in an online Pre-Departure Orientation. The purpose of this orientation is to address significant health, safety, cultural adjustment, and re-entry issues related to any international experiences. Site specific orientation is by your faculty site leader or sponsoring program/institution. View the Pre-Departure Orientation
Health Insurance
All students participating in international experiences are required to be enrolled in international health insurance
- Faculty-Led Program Participants - The Office of Study Abroad will enroll all faculty-led program participants in Highway to Health Insurance and provide cards to participants prior to departure.
- Exchange or Affiliated & Other Program Participants - Students are required to purchase Highway to Health insurance OR proof of insurance that has coverage that is equal to or better than coverage offered through Highway to Health. Visit www.uhs.umich.edu/tai to enroll.
Travel Registration
All students participating international experiences are required to register their travel details in the UM Travel Registry. In the event of an emergency, the Travel Registry will help the University get in touch with you, and will support your ability to reach us to get urgent safety, health, security, and related information.
- Faculty-led Program Participants - The Office of Study Abroad will register all faculty-led program participants
